The waivers listed below need to be filled out, signed (signature for/by player on IYSA waiver plus parent/guardian) and turned in this Saturday at Hassert Park. Travis Hughes, Becky Tomlinson, or Bridget Wisz will be at the picnic table (near the parking lot) to accept the forms.
